DAGİ DATA SUBJECT APPLICATION FORM

DAGİ Giyim Sanayi Ve Ticaret Anonim Şirketi (“DAGİ”) has prepared this Application Form in order to evaluate applications made by data subjects within the scope of Article 11 of the Law No. 6698 on the Protection of Personal Data (“KVK Law”) more quickly and accurately.

Rights of the Data Subject

Before filling out this Application Form, you can read the DAGİ Personal Data Processing Clarification Text available at www.dagi.com.tr to obtain information about your rights specified in Article 11 of the KVK Law and about DAGİ’s data processing activities.

Application Method

If you wish to exercise any of your rights specified in Article 11 of the KVK Law, you must fill out this Application Form and, together with the attachments specified under section IV,

1. 1. Submit a wet-signed copy of the form by applying in person to Merkez Mh. Birahane Sk. Koç Plaza No.3/1 34281 Bomonti-Şişli/İSTANBUL or
   2. Send a wet-signed copy of the form to Merkez Mh. Birahane Sk. Koç Plaza No.3/1 34281 Bomonti-Şişli/İSTANBUL via notary notification or
   3. Send the form to the electronic mail address dagi@hs02.kep.tr with your secure electronic signature.

I think my personal data processed by DAGİ is analyzed exclusively by automatic systems and that this analysis resulted in a negative consequence against me, and I object to this result.

I demand compensation for damages caused by unlawful processing of my personal data.

Please specify why you believe your personal data was unlawfully processed, the unlawful processing activity and the damage you suffered in the blank space at the end of this table or in an attachment to the Form. You may also submit Personal Data Protection Board or court decisions supporting your request with the Form.

V. Conclusion of the Application and Notification of the Result

DAGİ will evaluate and conclude your request within thirty days at the latest, depending on the nature of the request. Positive or negative responses to your request may be notified to you in writing or electronically. If you have a preference for the result of the application to be sent by mail or electronic mail, please specify this preference along with the address/email address to which the mail/electronic mail will be sent below:

Your requests will generally be concluded free of charge; however, if responding to your request requires an additional cost, fees may be charged within the limits set by the relevant legislation.

If DAGİ needs additional information to conclude the request or if it cannot verify that the application is made by the data subject, DAGİ will contact you through the contact information you provided in this Form or the contact information available at DAGİ.
